Identificamos as bases políticas, econômicas, ideológicas, sociais, institucionais e assistenciais que sustentam a economia política do setor saúde no contexto neoliberal no Brasil.

Based on critical thinking in health and the political economy of health, this thesis brings together studies that seek to update the understanding of healthcare in Brazil, considering that health policies are historically determined, politically instituted, socially produced and related to production and social reproduction. In Part I, after reviewing the relations between health systems, neoliberalism and financialization, the insertion of Brazilian companies in the health sector in the process of financialization is analyzed. Fifty-eight were selected and quantitative and qualitative information was collected from various sources between 2008 and 2017. The results are presented in two articles and in an exploratory analysis of the data gathered in the BDES-A database. Changes are observed in large companies such as rapid expansion and capitalization, increasing external investments, change in ownership structure, diversification, new strategies of competition, financing, organization and governance. The active role of the companies in the integration of the Brazilian health sector to financialization is demonstrated. Part II analyzes the formation, structural characteristics, and contradictions of the contemporary Brazilian health system, shaped by the health reform and the disputes established after the transition to democracy and neoliberalism. The implementation of the Unified Health System (SUS) and the changes in the relations between state, market and social classes between 1988 and 2016 were captured in the trajectory of processes such as financing, universalization, decentralization, marketization and segmentation. We identify the political, economic, ideological, social, institutional bases that underpin the political economy of the health sector in the neoliberal contexto in Brazil.
